This is to ensure that you are medically fit to drive passengers.
The form needs to be filled in by a registered medical practitioner who has access to your ‘FULL’ medical records therefore. Summary print outs will not be accepted. This can be requested from your NHS GP surgery. Your full medical notes should include all hospital and clinic letters if you suffer from any medical conditions. If you are unable to provide these then this will be documented in the declaration section of the form.
Medical conditions which could affect your application: Insulin treated diabetes, Epilepsy, Poor Vision, History of severe mental health problems (including drug or alcohol abuse), Heart problems, Neurological or neurosurgical disorders (strokes, blackouts, head injuries), Certain prescribed medication.
PCO Medical Checks will be held at our main office in central London. Please arrive 5 minutes prior to your appointment.
There is no definitive yes or no whether your application will be accepted as each case is dealt with on an individual basis by TFL.

- Photo ID
- TfL Medical Form
- Medical Notes or Medical Record from your GP
